The post holder will work as part of the general practice nursing team, providing nursing care to patients registered with the GP practice(s) within the PCN. The post holder will hold their own clinical caseload to support the needs of patients registered with the Practice(s) and to compliment the needs of the nursing team. This will include undertaking activities as outlined in the current Network Contract and associated direct enhanced services, such as supporting Personalised Care Support Pla

The purpose of the role is to provide support and technical assistance across all areas of the company’s IT environment. The role will be key to ensuring that adequate levels of support are maintained across the business whilst various projects are completed successfully.
